This role involves independent management of a caseload, utilizing advanced counseling, assessment, and decision-making skills to address the unique needs of each individual or family. The Chaplain ensures that the emotional and spiritual needs of patients are met while collaborating with interdisciplinary teams to promote holistic care.Our Hospice Care Mission is to provide compassionate and quality end-of-life care and grief support to patients and their families. Our Home Health Care Mission is to assist patients in achieving the highest level of potential in their day-to-day activities by providing professional and paraprofessional services in their homes. RequirementsJob Pay:$20 - $30 an hour$30- $40 per visitAmerican Hospice and Home Health Services is seeking a Part-Time Spiritual Chaplain/Counselor to serve in Stanislaus County.Negotiable salary and hourly pay rates depending on experience. Responsibilities andDuties:Patient and Family Care:• Provides spiritual and emotional support tailored to the beliefs and preferences of patients and families. • Conducts initial and ongoing spiritual assessments to identify needs and develop individualized care plans. • Offers grief counseling and bereavement support to families and caregivers during and after a patient’s journey. • Acts as a liaison between the patient’s faith community and the care team, when requested.• Coordinates with the interdisciplinary team to address spiritual and emotional goals in the overall care plan, in a meeting every other week. Documentation and Compliance:• Maintains accurate records of spiritual care services, including progress notes, communication logs, and updates to care plans. • Completes all documentation in accordance with agency policy and deadlines. • Participates in regular interdisciplinary meetings to review and revise care plans as needed. AdditionalDuties:• Offers rituals, prayers, and sacraments as appropriate to the patient’s faith tradition.• Participates in on-call duties as defined by the agency’s on-call policy. • Provides education to staff, patients, and families about grief, loss, and spirituality in the context of illness and end-of-life care. Licenses andQualifications:• Bachelor's degree in Divinity, Theology, Pastoral Counseling, or a related field (preferred). • Ordination or endorsement by a recognized faith tradition. • Minimum 1 year of experience in a spiritual care or counseling role, preferably in hospice or healthcare.• Current CPR certification. • Valid driver’s license and insured automobile, as required by state or agency guidelines.
