Remote MTM Pharmacist (Must be located in HI, TX, IL, FL, or PA)

Job Title:Remote MTM PharmacistPay: $54.05 an hr | Bi-weeklyJob Type:• Remote in either HI, TX, IL, FL, or PA and have an active, in good standing pharmacist license in that state. Shift Options:• Hiring for a M-F 8 hour shift starting between 9:00a - 11:00a CT. Training will be 8:00a-4:30p CT (approximately 2 weeks)• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needsRemote MTM Pharmacist Overview:• The role of the MTM Pharmacist is to deliver telephonic medication therapy management (MTM).Effective delivery of this bolthires telephonic based MTM service directly results in improved member care through the identification of medication related problems followed by the coordination of the resolution to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication related problems to members, caregivers, and prescribers. The MTM Pharmacist interacts successfully with patients, prescribers, nurse case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member attain their health related goals and help them on their path to better health.Pharmacists will also be trained to use the innovative skill of behavioral modification and interviewing, which are key drivers to positively impact patient health outcomes. • Perform telephonic comprehensive medication reviews, which includes but is not limited to ensuring a complete medication reconciliation is completed and performing clinical assessments on applicable conditions for each targeted member. • Identify, document, and facilitate resolution of identified medication related problems. • Create a written summary (patient takeaway) of each member interaction written at an appropriate level of health literacy.• Create faxed materials intended for prescriber offices to facilitate resolution to medication related problems. • Build and maintain positive relationships with internal and external customers including physicians and nurses. • Other responsibilities as directed by supervisor and/or manger. Remote MTM PharmacistQualifications:• Bachelor’s Degree in Pharmacy or PharmD from an accredited school of PharmacyApplicant must reside and have an active pharmacist license in either HI, TX, IL, FL, or PAPreferred:Previous pharmacy experience delivering MTM, specifically using the Outcomes MTM Connect platform.Basic computer skills and proficiency with bolthires Office products. Strong organizational skill, interpersonal skills, and detail orientated. Experience maintaining focus and meeting goals in a work from home environment.
