Remote (Kentucky) Clinical Pharmacist
Location: REMOTESalary: $60.00 USD Hourly - $62.00 USD HourlyDescription: Care Manager – Clinical Pharmacist (Patient Care)Location: FullyRemote (U.S.) Type: ContractAbout the RoleWe are seeking Clinical Pharmacists (Care Managers) to join our Clinical Engagement Services team. In this role, you will provide telephonic consultations, conduct comprehensive medication reviews, and support disease state management. You’ll collaborate with healthcare providers, document patient care, and contribute to improving clinical programs.Key Responsibilities• Conduct telephonic consultations and schedule pharmacist appointments with members enrolled in clinical programs. • Review prescription claims data to identify gaps in care, drug interactions, high-risk medications, and opportunities for therapeutic or generic substitution. • Assess medication regimens for unnecessary or ineffective therapies, inappropriate dosing, and adverse drug reactions. • Provide patient education on medications and disease states. • Document patient care notes and recommendations clearly and professionally.• Stay current with emerging clinical trends and participate in departmental meetings. • Meet productivity and performance expectations set by management. • Support special projects as assigned by the Operations Manager. Required Qualifications• Bachelor’s Degree in Pharmacy or PharmD. • Current and unrestricted Pharmacist license in Kentucky. • Strong knowledge of disease state management guidelines and the 5 Core Elements of MTM Services:• Medication Therapy Review (Comprehensive & Targeted)• Personal Medication List (PML)• Medication-Related Action Plan (MAP)• Interventions and Referrals• Documentation of Services and Follow-up• Experience working remotely and independently in a cross-functional team environment.• Strong organizational skills and ability to multi-task across multiple platforms. • Competent use of technology and MTM platforms. • Flexibility to work rotating shifts between 8:00am – 8:00pm CST, Monday–Friday (occasional weekends as needed). Preferred Qualifications• PharmD degree. • Bilingual fluency in medical terminology (Cantonese, Korean, Mandarin, Russian, Spanish, or Vietnamese). • 2+ years of pharmacy experience in retail, community, or PBM (new graduates with experiential rotations considered).• Additional pharmacist licenses in AL, ID, LA, MD, MN, OR, TN, or VT. • Prior call center experience. • ACPE MTM Specialist certification. • Certified Geriatric Specialist. • Board Certified Pharmacist.
