This individual is expected to provide high level support to medical virtual care providers, as well as assisting patients in navigating virtual care.This position is 0.5 FTE virtual care navigation/coordination. The other 0.5 FTE will be spent in the Customer Service Center. Essential Duties and Responsibilities• Serve as a liaison between virtual care providers and patients before, during, and after their virtual appointments. • Provide or arrange for technical assistance to patients on how to navigate PVCHC’s virtual care platforms. • Provide ongoing support, education, and training to the medical care teams on any updates to the virtual care process and procedures.• Assist providers during virtual care appointments to navigate clients and team members to virtual waiting rooms. • Call patients prior to their appointment (or during scheduling) to confirm necessary technology is in place and educate patients on virtual care process• Coordinate virtual care appointments• Enroll patients in the patient portal• Provide excellent customer service to patients and care teams. • Prepare and maintain charts, records, logs, and reports• Maintain all data entry in a timely manner.• Assist in completing reports regarding patient outreach opportunities, pre/post visit surveys, number of virtual care services provided and follow-up needed• Aids in evaluating virtual care platforms (such as patient portal, telehealth platform, remote patient monitoring, etc.) and developing efficient workflows• Participates in virtual care PI projects. • Supports and engages in training of care team members in virtual care modalities. • Engages effectively with team members outside of medical and supports overall integration of Peak Vista systems, particularly between medical, behavioral health, and dental clinical team.• Exhibits positive leadership skills, actively striving to improve the patient and staff experience. • Demonstrates exceptional communication skills. • Exhibits a patient and compassionate disposition. • Lives the PVCHC core values. • Other duties as assigned.
