Remote Patient Monitoring Dietician – ClassetLocation:Remote (Work from Anywhere in the U.S.)Schedule: Full-Time (Monday–Friday, 8:00 AM – 5:00 PM)Pay Rate: $27 – $30 per hourEmployment Type: Full-TimeOverview: We are seeking a skilled and compassionate Registered Dietitian to join our Chronic Care Management team. This is a remote, work-from-home role designed to provide ongoing dietary counseling and support to patients using weight management medications. The ideal candidate will excel in virtual patient engagement, using data from remote monitoring systems to guide personalized nutrition plans and conduct telehealth coaching sessions.Responsibilities• Remote Patient Monitoring: Track and interpret patient scale readings and other relevant health data. • Virtual Coaching: Conduct engaging one-on-one coaching sessions, providing personalized dietary education and motivation. • Customized Diet Planning: Develop tailored meal plans for patients, particularly those on medications such as Ozempic. • PatientEducation: Educate patients on healthy eating habits, nutrition, and lifestyle changes to support chronic care goals. • Collaboration: Work closely with physicians, pharmacists, and healthcare teams to ensure coordinated patient care.• Documentation: Maintain accurate records of patient interactions, progress, and dietary plans using EMR platforms.
