You'll utilize your advanced clinical judgment and critical thinking skills to facilitate care planning, coordination, and collaboration with healthcare providers, ensuring optimal outcomes and enhancing the overall wellness of our members. Position Summary:The Pediatric Care Manager RN will support a population of medically complex pediatric members, with a focus on those receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services.This role requires a strong foundation in pediatric patient care, case management, and collaboration with interdisciplinary care teams. You'll work closely with members, families, and healthcare providers to develop and implement personalized care plans, addressing medical, behavioral, and social needs. Key Responsibilities: Conduct comprehensive assessments of members' needs, utilizing clinical tools and data review to inform care planning and coordination. Develop and implement proactive care plans, addressing short-term and long-term goals, and facilitating collaboration with healthcare providers and community resources.Provide crisis intervention, support, and follow-up to members experiencing behavioral health or medical crises, ensuring timely access to necessary services and treatment. Apply clinical judgment and expertise to influence care decisions, promote optimal outcomes, and address complex health and social indicators. Collaborate with care teams, including medical directors, social workers, and other healthcare professionals, to overcome barriers and achieve member-centered goals. Requirements: Active, unrestricted RN licensure in the state of Florida.2+ years of experience in pediatric patient care, preferably in a hospital setting, ambulatory care, or alternative care environment. Willingness to travel 10-20% of the time for face-to-face member visits, with reliable transportation and mileage reimbursement. Proficiency in MS Office Suite applications, including Teams, Outlook, Word, and Excel. PreferredQualifications: Experience with members receiving private duty nursing, PPEC, Medical Foster Care, or nursing facility services. Case management experience, preferably in a pediatric population.Certified Care Manager (CCM) certification. Managed care or utilization review experience.
