Nurse Practitioner in Tulsa, OK- Hybrid Remote
About the positionResponsibilities• Support complex patients in navigating the healthcare system. • Deliver holistic, integrated care to improve quality of life for kidney patients. • Complete comprehensive health assessments. • Manage complex patients to reduce hospitalizations and improve health outcomes. • Coordinate and facilitate quality, consistent, cost-effective care. • Work autonomously and in collaboration with healthcare team members. Requirements• Minimum of two (2) years' experience as a Nurse Practitioner (NP).• Current unrestricted NP license to practice in the state. • Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire. • Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification. • Intermediate computer skills and proficiency in MS Word, Excel & Outlook required. • Home Office, High Speed Internet and Reliable Transportation required. Nice-to-haves• Preferred experience in Value Based Care or Population Health. • 5+ years' experience as a Family Nurse Practitioner (FNP) or in Internal Medicine, Cardiology, Nephrology, or Case Management.• Spanish-speaking preferred in some markets. • Experience in managing chronic diseases (CHF, diabetes, hypertension).
