As part of our commitment to innovation and excellence, we are expanding our Chronic Care Management (CCM) program and are seeking a Registered Nurse (RN) to support our care management team remotely.If you have a passion for patient care and experience in chronic care management, we’d love to hear from you!Key Responsibilities:

• Assist patients by providing information, coordinating services, and offering support remotely.
• Conduct outreach to patients for follow-ups, wellness checks, and care coordination.
• Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems.
• Support physicians and care managers by relaying patient updates, scheduling appointments, and ensuring smooth care transitions.
• Educate patients on managing chronic conditions and adhering to treatment plans.
• Maintain patient confidentiality and comply with HIPAA and healthcare regulations.
• Stay updated with industry best practices in telehealth and chronic care management.

Qualifications:

• Registered Nurse (RN) license in New York State
• 2-3 years of experience in a medical office, preferably in chronic care management or telehealth
• Proficiency in EMR systems and medical terminology
• Strong verbal and written communication skills
• Bilingual in Spanish highly preferred
• Experience working in a remote healthcare setting is a plus
