Note: The job is a remote job and is open to candidates in USA. Zócalo Health is a tech-enabled, community-oriented primary care organization serving historically underserved populations. The Director, CHW Operations will lead and scale the Community Health Worker function, focusing on building high-performing operations and ensuring effective integration within care teams.
Responsibilities
- Lead all Community Health Worker operations across markets, including workforce planning, deployment, productivity, and performance management
- Establish consistent standards for home visits, community-based interventions, member engagement, outreach, and field documentation
- Build management systems that support coaching, accountability, quality, and professional development across the CHW organization
- Partner closely with Nurse Practitioners, RN Care Managers, Behavioral Health Case Managers, and Care Coordinators to ensure seamless integration within care teams
- Monitor operational performance and identify opportunities to improve member engagement, retention, and outcomes
- Support expansion into new markets while maintaining consistency of service delivery and operational excellence
- Serve as the primary leader responsible for the effectiveness and scalability of the CHW function
Skills
- 8+ years of healthcare operations leadership experience
- Experience leading Community Health Workers, care coordinators, patient navigators, outreach teams, or other community-based healthcare workforces
- Experience building and managing distributed field teams
- Strong understanding of Medicaid populations and social determinants of health
- Demonstrated success developing frontline leaders and improving operational performance
- Excellent communication, coaching, and change management skills
- Experience with ECM, Community Supports, CalAIM, or other Medicaid care management programs
- Experience working with culturally diverse and historically underserved populations
- Multi-state operational experience
- Experience scaling community-based healthcare teams during periods of growth
- Bilingual Spanish/English strongly preferred
