REQUIRED EDUCATION and/or EXPERIENCE:

• MD or DO degree is required.
• Providers to be domiciles in Florida or within 50 miles of the Florida border.

CERTIFICATION, LICENSURE, and/or REGISTRATIONS:

• Board eligible is required.
• Board certified and trained in Psychiatry is required within 5 years of start date.
