Involves 75% travel to member homes, ALFs, and SNFs.Coordinates case management for Medicaid Long Term Care members, including assessments and care plans. Utilizes critical thinking, motivational interviewing, and negotiation skills to support member health outcomes. Ensures regulatory compliance, addresses quality-of-care concerns, and maintains accurate documentation.
