Job Title: Care Experience Navigator - Spanish speaking-_ (Independent Contractor / 1099)_Overview:LifeChef Health, and its wholly owned dietitian telemedicine subsidiary (On-Point Nutrition), is the only fully integrated platform for value-based nutritional intervention, delivering measurable outcomes for patients with, or at risk of, chronic conditions. Our platform combines Medical Nutrition Therapy, Medically Tailored Meals,Remote Patient Monitoring, and Digital Engagement into one seamless experience, scalable for care teams, trusted by payers, and personalized for patients.The Care Experience Navigator is the primary point of contact for patients/members throughout their journey with LifeChef Health—from initial referral through program completion. This role guides individuals through program options while providing continuous support to ensure a seamless and supportive experience. The ideal candidate is compassionate, detail-oriented, and thrives in a fast-paced environment. New responsibilities and tasks, in addition to changes to existing ones, may be made as deemed necessary and prudent by management and authorized by the Executive Team.Key Responsibilities:Patient/Member Engagement• Serve as the first point of contact for prospective and new patients/members via video, phone, email, or portal. • Build rapport to create a welcoming, empathetic experience for all enrolled individuals. • Clearly communicate LifeChef Health’s service offerings, provider availability, insurance coverage, and next steps. • Follow up with potential patients/members to support completion of intake forms and scheduling. • Other duties as neededIntake Coordination• Collect and verify demographic, insurance, and referral information.• Ensure all intake documents, consent forms, and authorizations are completed accurately. • Coordinate insurance eligibility and benefits verification as needed. • Enter and maintain complete, accurate records in the EMR system. • Other duties as neededOnboarding & Program Support• Schedule initial appointments and coordinate with clinical, nutrition, and administrative staff. • Provide patients/members with clear instructions for their first session and any necessary preparation. • Conduct orientation and onboarding sessions to help individuals set up and navigate the portal, digital tools, and meal profile management.• Monitor patient/member completion of onboarding tasks (e.g., device activation, form submission) and follow up on any pending items to ensure successful program engagement. • Other duties as neededOngoing Program Support• Maintain regular communication with individuals throughout their program• Assist with navigation, referrals, and appointment scheduling and rescheduling, as needed• Troubleshoot patient/member concerns and service-related issues (delivery, preferences, etc.)• Coordinate communication between patients/members and the multidisciplinary care team• Coordinate communication with referring providers• Follow up to encourage continued engagement and program completion• Other duties as neededDocumentation & Quality Improvement• Document all patient/member interactions accurately and timely in the EMR system• Conduct midpoint and post-program satisfaction surveys• Identify barriers and escalate concerns to appropriate team members• Participate in team meetings and contribute to process improvements, as needed• Adhere to HIPAA, company policies and procedures, and confidentiality requirements.Qualifications:Required:• Associate’s degree in healthcare administration or related field. • 1+ year of experience in healthcare, medical office, or patient/member services. • Excellent interpersonal and written/verbal communication skills. • Strong organizational skills and attention to detail. • Experience in EMR systems and digital platforms. Preferred:• Bachelor’s degree in healthcare administration, nutrition, public health, or related field. • Familiarity with care coordination, case management, or patient/member navigation• Experience in nutrition, behavioral health or specialty care settings.• Bilingual (Spanish) highly preferred.
